Notice of Privacy Practices
Healthaide Medical Group P.C.’s HIPAA Notice of Privacy Practices: how your medical information may be used and disclosed, and your rights.
Healthaide Medical Group P.C.
Effective date: October 25, 2026
This notice explains how Healthaide Medical Group P.C. ("Medical Group," "we," or "our") may use and disclose your protected health information ("PHI"), your rights regarding that information, and how you can exercise those rights. Please review it carefully.
The Medical Group provides healthcare through licensed clinicians to patients who may have enrolled through Aide Health. Healthaide LLC supplies the software platform and administrative support and performs appropriate functions on behalf of the Medical Group as a business associate. Aide Health, LLC, doing business as Aide Health, operates the branded Website and assists with customer-facing services. Medical records are maintained under the responsibility of the Medical Group.
This HIPAA notice is different from the Website operator's Privacy Policy and consumer health data policies, which address information outside the scope of this notice.
Your rights
You have the right to:
- Inspect or obtain an electronic or paper copy of your medical record and other PHI available to you under HIPAA. We generally respond within 30 days. A permitted, reasonable, cost-based fee may apply.
- Request corrections to information you believe is inaccurate or incomplete. We may deny a request for a legally permitted reason and will explain a denial in writing, generally within 60 days.
- Ask us to communicate with you in a particular way or at a different address. We will accommodate reasonable requests.
- Ask us to limit certain uses or disclosures. We are not generally required to agree, except that if you pay fully out of pocket for a service, we must generally honor your request not to disclose that service to a health plan for payment or healthcare operations, unless the law requires disclosure.
- Request an accounting of certain disclosures made during the preceding six years, subject to legal exclusions. One accounting in a 12-month period is free; an additional accounting may involve a permitted cost-based fee.
- Obtain a paper copy of this notice, even if you previously agreed to receive it electronically.
- Have a legally authorized personal representative exercise your rights, subject to verification of that authority.
- File a privacy complaint without retaliation.
To request records or exercise a right, contact the Medical Group Privacy Officer using the routing contact information listed below or your secure patient portal. We may ask for information reasonably necessary to verify your identity or authority.
Your choices
Where the law gives you a choice, you may tell us whether to share information with relatives or others involved in your care or payment, or in disaster-relief situations. In some circumstances, when you cannot communicate your preference, we may share information if permitted by law and reasonably necessary for your care or safety.
We generally need your written authorization before using or disclosing PHI for marketing as defined by HIPAA, selling PHI, or using most psychotherapy notes for purposes requiring authorization. You may revoke an authorization in writing, except to the extent we have already acted on it or another law permits the activity.
How we use and disclose information
We may use or disclose PHI without your separate authorization when permitted or required by law, including to:
- Treat you: evaluate symptoms, coordinate care, review tests, prescribe medication, and share relevant information with treating clinicians, pharmacies, and laboratories.
- Receive payment: process or document charges, payment, and related transactions.
- Run the Medical Group: provide quality oversight, credential clinicians, audit and secure systems, support patient services, and meet operational and compliance requirements.
- Work with business associates, including Healthaide LLC, under appropriate privacy and security obligations.
- Fulfill public health and safety duties, including legally required reporting, medication safety reporting, and preventing serious threats.
- Comply with legal obligations, authorized health oversight, judicial proceedings, lawful government requests, or workers' compensation requirements, subject to applicable safeguards.
- Respond when legally permitted to organ donation organizations, coroners, medical examiners, or funeral directors.
- Conduct legally permitted research under applicable authorization or approval requirements.
We apply any applicable federal or state law that provides stronger confidentiality protections, including rules concerning certain mental health, HIV, genetic, reproductive, or substance use records. A specific disclosure may require separate authorization or additional safeguards.
Special protection for substance use disorder records
If we receive or maintain substance use disorder patient records covered by 42 CFR Part 2, additional federal protections apply. We will not use or disclose those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and subpoena that satisfy applicable law. We obtain consent for other Part 2 uses and disclosures when required. If fundraising involving Part 2 records is ever proposed, we will provide the advance notice and choice required by law.
Our legal duties
We are required to protect the privacy and security of PHI, follow the notice currently in effect, and notify affected individuals of a breach of unsecured PHI when required by law. We will not use or disclose PHI for purposes outside this notice unless allowed by law or authorized by you in writing.
We may revise this notice. Changes may apply to information already maintained by the Medical Group as permitted by law. The current notice will be available on this Website and on request.
